This form is needed by leaders that are working with the youth.  This is a measure that we want to go through to help keep everyone safe and add peace of mind.

Permission Form - Winter Jam 2012

Please complete one per student participating.

Permission Form - Lock-in

You MUST bring this form signed by a parent to participate in the lock-in.  Bring friends but make sure they have a signed form too.

Permission Form - Mystery Trip

Each student must have a permission form signed by parent or guardian.

Permission Form - Table Rock Trip

Each student must have a completed form and payment ($30 per person, $20 for siblings).
